We are excited to announce that we are now offering the convenience of online registration through Genesis Parent Portal.  When you register through Genesis, the system keeps track of your information through your account. 

Please use this checklist and the information provided on this page to guide you through the registration process. You can access the physical exam forms and medical care plans here.

**Prior to registration it would be helpful to know your student's most recent physical exam date.**

REGISTRATION PROCESS:

Parents/Guardians should register by clicking on this link: 

Genesis Parent Portal

Follow these steps:

1. Log into parent access
    

2. Click Forms
    

3. Click Forms Library
    

4. Complete SPORT PARTICIPATION REGISTRATION (for current season)
    

5. Click LIVE LINK – complete (1-11) on Health Questionnaire (HHQ) if physical exam date is greater than 90 from the start of the sport season.
    

6. Fill in today’s date
    

7. Check confirm box
    

8. Click the green box to "Submit"
